The monthly income limits are as follows for 2020:
You may not have private, public, or self-purchased health insurance or benefits (including Veterans Administration, Medicaid or Medicare.) This also applies to dental services at the clinic even if your health coverage does not extend dental benefits.
First time patients or patients who have not been seen at the clinic in the last 5 years will need to fill out a patient information sheet as well as a healthcare application form. This usually takes about 30 minutes to complete.
• Current picture identification
• Social security card or number (not required for service)
• Verification of Income (pay stub, W-2 form, Tax return etc. from all working members of the house in which you live)
• Medical and/or Prescription history paperwork
The scope of care provided by the clinic is limited due to the volunteer nature of our clinic. Our staff will select patients for registration based on our ability to professionally meet their overall healthcare needs. We cannot guarantee any patient will be admitted for care and will do our best to refer you to appropriate resources.
Patients are not refused service based on race, religion, gender, sexual orientation, national origin, age, residency status, or for any other non-financial/non-medical reason.
If you do not speak English you need to bring a translator with you to your appointment (Si usted no habla Inglés que tiene que traer un traductor con usted a su cita.)
No appointment needed! Open every Tuesday and Thursday at 5:00pm. The only qualification is that you have no insurance and you make less then the eligibility requirements.
Please note the average wait time for evening clinic is 1 hour from registration to seeing a provider. Our volunteers work as fast as they can after already working all day so we ask for your patience.
To protect our patients, providers and staff every person is required to wear a mask while in the clinic.Patients will be pre-screened for COVID-19 symptoms. Only one person is allowed in the exam room at a time, additional people will have to wait outside.

Tuesday & Thursday evening walk-in clinic, check in at 5:00pm.
We are closed Fridays and holidays.

The Wellness Tree Community Clinic is committed to continuing to provide care for our community members during the Covid-19 challenges. In order to protect our patients and staff we will be screening for the virus prior to allowing them to enter the clinic. This screening consists of nine questions and a temperature check. Also, all persons will be required to wear a face mask while inside the clinic at all times. If you do not have a mask, we will provide one. Thank you for your understanding.
